A security guard at an East Village church slashed in the throat after asking a stranger to stop urinating on a parked car was back at work a day later — sporting a gruesome scar from the wound that nearly killed him.

John Mach, 57, complained when he saw the stabber urinating on a car parked outside the Church of the Immaculate Conception on E. 14th St. near First Ave. about 5:05 p.m. on Sunday, cops said.

“He was urinating and doing all sorts of nuisance,” Mach told the Daily News on Monday in an exclusive interview. “He was looking for problem. As a trained counselor, I can tell.”

Mach said before he was slashed, the attacker, who he had never seen before, had been threatening him and his co-workers with the box cutter used in the assault.

“My duty is to guard my church and keep everyone safe,” Mach said. “He did look like someone with a mental health problem and I did not want anyone to get in trouble, … He kept saying that he’ll slash me and my colleagues, but then he went away.”

The troublemaker soon returned but wearing a different jacket, according to Mach. On Monday, cops released surveillance footage of the on-the-loose suspect in the second coat.

“He was not wearing the red puffer jacket when he first threatened me and my colleagues,” Mach explained. “He was wearing a dark-colored jacket, jeans and a black cap. … After a while, he returned in the red jacket, which is why I could not recognize him.”

That gave the attacker the chance to launch a surprise attack against an unwary Mach.

“He just came and slashed my throat,” the victim said.

Mach ran behind the suspect for a block, to E. 13th St., while his throat bled from the wound. He wrapped the thermal shirt he was wearing around his neck as a makeshift bandage. His attacker got away, but only after being smacked by good Samaritans trying to slow his escape, according to Mach.

“I was not letting him go away like that after slashing me,” Mach said. “I tied a cloth around my neck to stop it from bleeding and ran after him. But he fled. I was told that he got hurt after fleeing, people hit him. Everyone knows me here, you know.”

Mach was admitted to Beth Israel hospital, where he received 16 stitches to the long gash running nearly the entire length of the left side of his throat.

“He was close to the arteries,” Mach said. The NYPD said Mach was initially in critical condition, but Mach said he isn’t ready for heaven yet.

“God does not want me there so soon — so here I am,” he said after returning to work Monday.

Cops are asking the public’s help identifying the suspect and tracking him down.
